Leveraging Technology to Reduce Implicit Bias and Improve Population Health Metrics

Description: Proactive Ochsner Encounters is a systematic approach which empowers all members of the clinic team to proactively identify and close gaps in patients' preventive cancer screenings and vaccinations. Currently, Primary Care is responsible for closing care gaps for our patients, but not all patients see their Primary Care Provider on a regular basis. The goal of the project is to improve our patients health status through increased compliance with preventive screening measures, specifically by engaging specialty care teams to assist in reminding patients during specialty visits that they are overdue for these screenings. To do this, the EMR was reconfigured to utilize a reminder for any patient who has an Ochsner Primary Care Provider and one of the following screenings overdue: mammograms, diabetic retinopathy exams, colorectal screenings, and flu, tetanus, pneumonia, and shingles vaccinations. This method not only engages the specialty clinics in Population Health, but also leverages the EMR to promote equality of care. In listening to our team's feedback and partnering with the experts in EPIC, we built a solution which functions on automation. At Registrations, a document will automatically print out that has the patient's overdue screening listed, as well as a brief description of why that screening is important and why early detection is key. Patients are able to review this while waiting to head back to the clinic exam room. Once in the exam room, EPIC built a function within the rooming tab which prompts the MA or Nurse to remind the patient they have an overdue screening. The providers also see this during their visit. If a patient would like to schedule the screening, they are able to do so as they leave the clinic. Utilizing EPIC and enhanced functionality, we are able to remind patients of these important screenings at each touch during a clinic visit. We currently are live with this proactive approach in our specialty clinics at one of our campuses at Ochsner Health System, and plan to be live in a total of 5 campuses by the end of 2019. We also have plans to engages our online portal (MyOchsner) and central scheduling, so that if patient are making appointment on their own online or if they are calling us to make an appointment, they will also be reminded of their overdue screenings.
