Live Well: Physical Exercise and Cognitive Training After Stroke

Description: Implementing care of stroke patients can be challenging in many ways.  Medication adherence, access to early intervention, dietary management, and many other tactics to improve health are hard to move from theory to practice.  In this session, we will take on the special problem of implementing exercise interventions to improve fitness after stroke. The literature indicates, strongly and with consensus across many disciplines, that exercise improves health after stroke.  However, most neurologists are not familiar with implementation science: this literature offers a roadmap toward optimal use of evidence-based care methods.  We will start by discussing specific exercise interventions, and the evidence and discipline-specific consensus documents supporting their use. Then, the emphasis of this session will be on obstacles to implementing these interventions in neurological care.  We will review recommendations on how to ensure that patients receive exercise treatment, based on current literature.  The last part of this session will be devoted to discussing cognitive interventions, which are promising in stroke, although not as far down the translational continuum.  We will discuss discipline-specific consensus guidelines for cognitive rehabilitation and treatments for three specific disorders: aphasia, limb apraxia, and spatial neglect.
